EXOSOMES ARE SIGNALLING MOLECULES, THE MESSAGES CELLS USE TO TELL EACH OTHER WHAT TO DO. They carry no cells and no DNA. What they carry are instructions, and after microneedling those instructions land in skin that is already primed to act on them.
Microneedling creates thousands of temporary microchannels and triggers the skin’s own repair response. For a short window afterwards the barrier is open and the skin is unusually receptive. Applying exosomes in that window is the whole point of pairing the two.

An exosome microneedling appointment takes 60 to 75 minutes including numbing. Here is the sequence.

Preparation (before your visit)
Stop retinoids and exfoliating acids 5 to 7 days before your appointment. Avoid excessive sun exposure and arrive with clean skin, no makeup.

Numbing (about 20 minutes)
Topical numbing goes on first and we let it work properly. This is the part most people are surprised by, because it makes the treatment itself straightforward.

Microneedling with exosomes (about 30 minutes)
Your provider passes the device at a depth chosen for your skin and the area treated, then applies the exosomes immediately, while the channels are still open.
