Sports Physicals Offered
At GENEVA, a sports physical is built to clear your athlete and protect them at the same time. Whether you need a school sports physical, a GHSA preparticipation form, a club or travel team clearance, or a college athlete exam, our evaluations give you the documentation you need and a real look at your athlete's health, in a thorough, unrushed visit.

Preparticipation Sports Physical
The full clearance exam for school, club, and travel teams, completed in a single visit with your forms signed before you leave.

Same Day Clearance Forms
School, GHSA, camp, league, and college forms filled out and signed on site, so a tryout or registration deadline never slips.

Cardiac Screening for Athletes
A focused heart history and exam by a physician, with an onsite EKG when your athlete’s history or symptoms call for one.

Orthopedic and Mobility Assessment
Joint, strength, and flexibility screening that catches the old ankle, knee, or shoulder problem before the season makes it worse.

Asthma and Breathing Evaluation
Evaluation for exercise related breathing trouble, so a young athlete is not quietly underperforming because of untreated asthma.

Team and Family Group Physicals
Several athletes seen back to back, whether it is siblings before a season or a roster that needs clearing on one afternoon.

Sports Physicals Without the Wait
At GENEVA, a sports physical is a real medical exam, not a signature at a folding table. Whether your athlete plays middle school, high school, club, travel, or college sports, you will be seen promptly in Alpharetta and leave with the required forms completed.
We review the medical and family history that matters most in young athletes, screen the heart, and check the joints that carry the load in your athlete’s sport. If something needs a closer look, we can run labs or an EKG onsite and explain exactly what comes next.
You will see the same physician who knows your family, which means changes from last season get noticed rather than missed. We are locally owned and just a mile from Avalon, serving Alpharetta, Johns Creek, Roswell, Milton, and Cumming.

Athlete and Family History
We review past injuries, concussions, asthma, medications, supplements, and any family history of heart problems or sudden cardiac events.
Vital Signs and Growth Check
Blood pressure, heart rate, height, weight, and vision, measured and compared against what is expected for your athlete’s age.
Cardiac History and Heart Exam
A focused evaluation for chest pain, fainting, palpitations, and shortness of breath during exertion, with an onsite EKG when the history calls for it.
Head to Toe Physical Exam
A full examination of the heart, lungs, abdomen, skin, and neurological function, not only the parts a form asks about.
Musculoskeletal Screening
Joint stability, strength, flexibility, and range of motion, so an old ankle, knee, or shoulder problem is caught before the season makes it worse.
Concussion and Return to Play Review
A careful look at any history of head injury, with clear guidance on symptoms to watch for and when it is safe to return to competition.
Screenings and Testing as Needed
Lab work, iron and vitamin D testing, or a breathing evaluation when your athlete’s history or symptoms point that way.
Forms Completed and Signed
School, GHSA, camp, club, and college forms filled out and signed during the visit whenever possible, so nothing delays registration.
