Telehealth Informed Consent
Telehealth lets you receive care from a GENEVA clinician by secure video or phone instead of in person. Before your first telehealth visit, please read this consent. By scheduling or joining a telehealth visit, you confirm that you have read and understood it and agree to receive care through telehealth.
What to Expect
- Your clinician will review your history and discuss your concerns by video or phone and may review photos, records or test results you share.
- Your clinician will decide whether telehealth is appropriate for your concern. Some conditions, examinations, tests and treatments require an in-person visit, and you may be asked to come to our office, go to a laboratory, or see another provider.
- Information from your telehealth visit becomes part of your medical record, and the same privacy protections apply as for an office visit. See our Notice of Privacy Practices.
Location and Licensure
GENEVA offers telehealth only to patients who are physically located in Georgia at the time of the visit. You must be in Georgia for every telehealth visit and tell us where you are at the start of each visit. If you are outside Georgia, we will need to reschedule. Your clinician may need to verify your identity.
Prescriptions and Controlled Substances
A clinician may prescribe medication after a telehealth visit only when it is medically appropriate. Controlled substances, such as ADHD stimulants and testosterone, are prescribed only as permitted by federal (DEA) and Georgia law, which may require an in-person evaluation, identity verification, prescription-monitoring checks, laboratory tests and regular follow-up visits. We cannot promise that any medication will be prescribed.
Benefits and Risks
Benefits include easier access to care, less travel and time away from work, and more convenient follow-up.
Risks include:
- the clinician cannot perform a hands-on physical examination, and information shared may not be enough to make a diagnosis or treatment decision;
- poor video, audio or image quality, or a dropped connection, may delay care or require an in-person visit;
- despite the security measures we use, electronic communication carries a small risk of interception or unauthorized access; and
- in rare cases, a limited examination may lead to an error in judgment.
Your Responsibilities
- Join from a private place where you are comfortable discussing your health, using your own device and a secure internet connection.
- Give complete and accurate information about your health, medications, allergies and location.
- Do not record the visit without your clinician’s permission. We do not record telehealth visits.
- Follow up as your clinician recommends, including in-person visits and tests.
Emergencies
Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room. If the connection is lost during a visit, your clinician will try to call you back at the number on file. If your clinician believes you need emergency care, they may contact emergency services at your location.
Your Rights
- You may decline telehealth and ask for an in-person visit at any time, without affecting your right to future care.
- You may withdraw this consent at any time by telling us.
- You have the same rights to access your records and to privacy as with an in-person visit.
