Payment, Cancellation & Membership Policy
Self-Pay Practice
GENEVA is a self-pay practice. We do not bill health insurance, Medicare or Medicaid, and we do not submit claims on your behalf. On request we will give you an itemized receipt that you may submit to your insurer, health savings account (HSA) or flexible spending account (FSA); we cannot guarantee that they will reimburse you. You have the right to a Good Faith Estimate of expected charges before your visit.
Prices and Payment
- Prices are shown when you book or are quoted by our team. Prices for some treatments depend on your treatment plan (for example, the number of units or syringes used) and will be confirmed before treatment.
- Payment is due at the time of service unless you have a membership or prepaid package.
- We accept major credit and debit cards. HSA and FSA cards are accepted when the card issuer approves the charge; if a charge is declined (for example, because of the account balance or the issuer’s rules), another form of payment is due.
- Deposits. Some services, such as primary care visits and dermal fillers, require a deposit or a card on file when you book online. Deposits are applied toward the cost of your treatment. Free consultations do not require a card.
- Prescription medications and laboratory tests may be billed separately. Your clinician or our team will tell you the cost before you commit.
Cancellations, Late Arrivals and Missed Appointments
- Please give us at least 24 hours’ notice to cancel or reschedule, by phone, text, email or through your booking confirmation.
- Appointments cancelled with less than 24 hours’ notice, or missed without notice, may be charged a $75 late-cancellation fee, and any deposit may be applied to that fee. We may waive the fee at our discretion, for example for emergencies and circumstances outside your control.
- If you arrive more than 30 minutes late, we may need to shorten or reschedule your appointment, and the late-cancellation fee may apply.
Refunds
- All sales are final. We do not offer refunds for services performed, prepaid packages, memberships or retail products. We may, at our sole discretion, offer a credit or make an exception in individual cases; doing so does not change this policy.
- If we are unable to provide a service you have prepaid for, for example because your clinician determines it is not medically appropriate for you, we will refund or credit the unused portion.
- Results vary and are not guaranteed. If you have a concern about a treatment, please contact us within 14 days so your clinician can evaluate it. Any touch-up or adjustment is at your clinician’s discretion.
- Retail products cannot be returned or exchanged, except where a product is defective or as otherwise required by law.
- Promotional credits have no cash value. Gift cards are not refundable or redeemable for cash except as required by law.
Memberships
- Automatic renewal. Memberships are billed monthly to the card you provide, at the price shown when you join, and renew automatically every month until you cancel.
- Minimum term. Each membership has a minimum term of 3 monthly payments. After the minimum term you can cancel at any time.
- How to cancel. After the minimum term, cancel by calling or texting (470) 704-9687, emailing careteam@geneva-med.com, or telling us at the front desk. Cancellation takes effect at the end of the billing month you have already paid for, and we will confirm your cancellation in writing.
- Price changes. We will give you at least 30 days’ notice before changing your membership price. You may cancel before the new price takes effect.
- Member benefits. Any rules for using or carrying over monthly benefits are set out in your membership agreement. Unused benefits end when your membership ends, and member pricing applies only while your membership is active.
- Your membership agreement, which you accept when you join, contains the full terms.
